Preparing for Your Appointment

To ensure a smooth check-in and establish your permanent medical record, please review the steps below prior to your visit.

Please complete the following required forms prior to your scheduled arrival:

Please have the following items ready upon arrival at our front desk:

  • Valid government-issued photo ID (driver’s license, state ID, or passport)

  • Current health insurance card(s)

  • Primary care physician (PCP) referral / authorization (if required by your insurer)

  • Complete list of current medications, supplements, and dosages

  • Relevant previous diagnostic records, MRI / CT imaging discs, or lab reports

What to Expect During Your Evaluation

To help us accurately assess your condition and save time during your visit, please be prepared to discuss your symptoms:

  • Primary Concern: What is bringing you in?

  • Timeline: When did it start, and was the onset sudden or gradual?

  • Pattern: Are symptoms constant or intermittent? How often do they occur, and how long do they last?

  • Associated Symptoms: Have you noticed any other changes or related issues?

  • Recent Episode: When did it last occur?

Note: Depending on your symptoms, Dr. Albakri may order specialized diagnostic testing—such as blood work or brain and vascular imaging—to tailor your treatment plan.

Our Policies

Clinic Policies & Guidelines

Please review our office policies prior to your visit. These guidelines help ensure a smooth experience and protect your privacy and health.

1. Privacy, Dignity & Confidentiality

  • Respectful Care: You have the right to considerate, courteous care that respects your personal dignity.

  • Confidential Records: Your medical history, communications, and records are kept strictly confidential under federal privacy laws, except where disclosure is legally mandated by law (such as reporting public health hazards or suspected abuse).

2. Insurance & Payments

  • Referrals & Authorizations: If your insurance plan (e.g., HMO or POS) requires a referral from your Primary Care Physician (PCP), please ensure this is secured before your visit.

  • Copays & Balances: Copayments, deductibles, and any previous unpaid balances are due at check-in.

  • Medicare Patients: If Medicare is your primary insurance and you do not have supplemental (secondary) coverage, the 20% Medicare allowable coinsurance fee is collected at each appointment.

3. Cancellations & Rescheduling

  • 48-Hour Notice: If you need to cancel or reschedule, please notify our office at least 48 hours in advance.

  • Cancellation Fee: A $40.00 fee will apply to missed appointments or cancellations made with less than 48 hours notice.

4. Prescriptions & Refills

  • In-Person Only: For your safety and ongoing clinical monitoring, prescription refills are provided exclusively during your scheduled doctor visit.

  • We cannot process refill requests over the phone or via pharmacy call-ins.

5. Test & Lab Results

  • In-Office Discussion: In compliance with federal patient privacy laws, test and lab results cannot be given over the phone.

  • Dr. Albakri will review and explain your results in detail with you during your scheduled follow-up visit.

6. Medical Records Requests

  • Written Authorization: To obtain copies of your medical records, a signed written release from the patient or legal guardian is required.
